Medetomidine hydrochloride is a potent, highly selective alpha2-adrenergic receptor agonist primarily utilized in veterinary medicine as a sedative, analgesic, and muscle relaxant for dogs and cats. By stimulating central alpha2-adrenoceptors, it inhibits the release of norepinephrine, inducing dose-dependent sedation and profound chemical restraint to facilitate clinical examinations, minor surgical interventions, and pre-anesthetic management. Due to its potent peripheral vasoconscriptive properties, its administration typically causes a transient rise in blood pressure followed by a compensatory bradycardia. Its sedative and physiologic effects can be rapidly and completely reversed using the specific antagonist atipamezole. While not approved for therapeutic human use, it has recently emerged as a dangerous potent adulterant in the illicit synthetic opioid supply.
